The Welburn Gourd Farm cordially invites you to complete the application process for the 14th Annual International Gourd Art Festival! With a limited number of Food Vendors, you are sure to have high sales and traffic to your food booth!

We strive for high quality and excellence in everything we do! And because of our careful selection process and "no competition" mentality for our food vendors, there is a high demand for the limited number of spaces.

Tents, tables, chairs, and screen walls are available for rent. The rental fee must be paid in full and sent with your application.

All applications must be postmarked no later than March 31st, 2010. Information Packets will be mailed to selected Food Vendors by May 1st, 2010.

• Application Checklist:

• Completed Application form, including your California Resale Number (permanent or temporary), San Diego County Health Permit Number, booth requirements, and any additional rentals you require.
  

• Complete list of all food and drink items and prices you intend to sell.
  

• Payment in full for Balance Due as determined by the Booth Rentals section of the Application Form, if applicable.

A couple of items to emphasize: We MUST have your California Resale Number and San Diego County Health Permit Number to allow you in the show. As we have been progressively growing in size and attendance, the State Board of Equalization and the Department of Environmental Health will be checking food vendors more closely. To apply for a Permanent or Temporary Resale Number, please contact the SBOE Information Center at 1-(800)-400-7115 as soon as possible to obtain an SBOE application. To apply for a Health Permit, please contact the Department of Environmental Health at 1-(800)-253-9933 to obtain a San Diego County Temporary Food Event Permit application.
